Role overview

 

We are looking for an enthusiastic and skilful person with bags of energy to join our successful and growing Shared Lives Service.

 

You will have a passion for providing the highest quality standards of care and be a proactive practitioner with the ability to work well within an established tight knit team.

Key responsibilities

  • To play an active part in the day to day work of the scheme, supporting Shared Lives carers and service users to help maintain successful placements where everyone can thrive
  • To undertake assessments of potential carers, supporting them through the application process and producing appropriate reports for the Approval Panel
  • To work in partnership with individuals, families, carers and other professionals to develop person centred care plans, with regard to relevant legislation and service requirements
  • To undertake regular monitoring and review activities to ensure the effectiveness of the support provided
  • To play an active role in maintaining and promoting the scheme, acting as an ambassador and role model at every opportunity

Key Requirements

To succeed in this role you will need as a minimum:

  • A determination to provide the best possible service and a willingness to fully embrace the ethos of the scheme
  • Experience of working with people with a learning disability and/or mental health conditions
  • A relevant professional qualification at NVQ Level 3 (or equivalent)
