The Oldham Health and Social Care economy has signed up to a new and ambitious vision for care which looks to ‘see the greatest and fastest possible improvement in the health and wellbeing of the Borough’s residents by 2020’.

The vision will be achieved through delivery of a health and social care system that is built upon sustainable financial and person-focussed models, with integration at the heart of health and social care service delivery.
To support the vision we have brought together a range of partners including for example Adult Social Care, Pennine Care NHS Foundation Trust, MioCare, primary care and the Oldham Clinical Commissioning Group to create a new integrated system approach– Oldham Cares.

Oldham Cares is the new way health and social care services will be delivered. It brings together social, health and voluntary organisations to share knowledge, resources and skills to deliver a better health and social care experience for our residents.

At Oldham Cares we are focussed on achieving positive outcomes for people with health and social care needs including: 
  • Supporting people to be more in control of their lives Having a health and social care system that is geared towards wellbeing and the prevention of ill health
  • Providing access to health services at home and in the community
  • Providing social care that works with health and voluntary services to support people to look after themselves and each other

We are looking for a number of passionate, enthusiastic and experienced social workers working across adults’ services. This could either be in one of our cluster teams (of which there are 5), our Multi-Agency Safeguarding Hub (MASH) or the Integrated Discharge Team at the Royal Oldham Hospital/ Fairfield General Hospital.

You will be qualified to degree level, have an appropriate post qualifying award such as consolidation module, practice educator stage 1 and/or 2, Best interest Assessor or equivalent with significant experience and be HCPC registered.

Your main duties will be to provide an assessment, support planning and review service, underpinned by professional knowledge and legislation in order to ensure that:

  • Significantly complex individuals are supported appropriately including undertaking mental capacity assessments and best interest processes 
  • Vulnerable adults are safeguarded and are supported to live well with risk;
  • Strength based conversations that result in person centered outcomes and support requirements are identified, and;
  • People are encouraged and enabled to live healthy and independent lives.
