Role Overview:

To manage and lead an integrated the Care Management and Review Team that sits alongside two busy Community Mental Health Teams. 
The Review team reviews all those open to the team who have 117 commissioned services, a proportion of which are in placements outside of Oldham. The Team also reviews those commissioned placements/ packages not subject to 117 that are open to the team.

In fulfilling its statutory role, the Manager will be responsible for ensuring continued close working with CCG colleagues. The manager will ensure all Local Authority Statutory duties are carried out within the team- Care Act , Sec 42 Safeguarding, Mental Health Act – Sec 117 , Mental Capacity Act and all other relevant legislation all relevant Council policies and procedures. The Team also operates within the Care Programme Approach and all Relevant Pennine Care Mental Health Foundation Trust policies and procedures.

 

This role requires the ability to manage and lead an integrated team. The successful candidate will be someone is resilient and able to respond in a measured way to competing demands on their time, knowledge and experience and that of the team. The Manager will provide supervision and support to practitioners both Social Workers and Nurses., including conducting formal supervision sessions and advising on complex practice issues within a legal and good practice framework.

The role requires the ability to utilise the coaching model of leadership and the ability to negotiate with others to resolve often complex practice challenges. The successful candidate will lead the team through a significant period of transformation across the Mental Health System in Oldham over the coming 3 years and be expected to take on development roles within the transformation work . The Team manager and is an important role within Leadership Team and will work closely with the PCFT Service Manager and the LA Head of Service Mental Health.

There will be opportunity to develop professionally, and be accountable for the delivery of, and promote, social work practice and expertise within the integrated Mental Health partnership while understanding and supporting health colleagues.

The successful candidate will represent the service at forums such as Safeguarding Adult Reviews, Mental Health programme Board and the Achieving Better outcomes Project.The role will require that on occasion you cover for the HOS or PCFT Service Manager. 

 

Key Responsibilities:

  • To screen referrals and allocate work to team members.
  • To review the work of the team- practitioners to ensure quality is maintained and all work is complying with relevant legislation.
  • The role support and offer advice on safeguarding concerns and ongoing work.
  • The role requires that the successful candidate act as a Safeguarding Adult Manager- chair and lead on safeguarding enquires. 
  • To ensure a strength based approach is applied to all areas of work within the team.
  • To ensure all commissioned services are best value and outcome focused and as far as possible co-produced with those using the service.
  • To ensure that data on the team’s performance is produced and supports service development.
  • To support the HOS to ensure that the teamwork within budget
