Contract Type: Permanent

Salary: Grade 7 / 8 (Point 33 – 42 £29,909 - £38,052 p.a.).  Please note that the Grade 8 salary is subject to an AMHP qualification.  

Hours: 36.66 per week

Based: Maple House, Hamilton Street, Oldham

Role overview

Working as a social worker and care co-ordinator for the integrated Community Mental Health Team Adults.  Undertaking assessments and reviews in line with The Care Act 2014 and in conjunction with the requirements of the Care Programme Approach. 

 

Key responsibilities

  • Undertake assessments of need in line with Care Act, Mental Capacity Act, Care Programme Approach and Mental Health Act (where applicable).
  • Co-produce care and support plans in partnership with service users, families and carers in line with the Care Act 2014, MCA 2005 and Care Programme Approach requirements.
  • Undertake risk assessments and formulation of Risk management plans in line with the requirements of the Care Programme Approach
  • Acting as Safeguarding enquiry officer and where appropriate undertake Safeguarding Enquiries in line with Oldham’s Council’s Multi-agency policies and procedures,
  • Work effectively as a key member of a multi - disciplinary team

Key Requirements

To succeed in this role you will need as a minimum:

  • To be registered with the HCPC as a qualified Social Worker
  • To have at least 2 years post qualifying experience in mental health or related area of practise
  • Excellent Communication and Interpersonal Skills
  • To be able to make good decisions based on available information; ensuring that children are appropriately safeguarded and their needs are met.
  • Have an ability to work effectively and contribute as part of a Team
  • Have a good experience of using Health and Social Care IT Recording systems and maintaining accurate records/information
