To support the vision we have brought together all the partners from Adult Social Care, Pennine Care NHS Foundation Trust, MioCare, primary care and the Oldham Clinical Commissioning Group to create a new organisation – Oldham Cares.

 

Oldham Cares is the new way health and social care services will be delivered. It brings together social, health and voluntary organisations to share knowledge, resources and skills to deliver a better health and social care experience for our residents.

  • Supporting people to be more in control of their lives
  • Having a health and social care system that is geared towards wellbeing and the prevention of ill health
  • Providing access to health services at home and in the community

Role overview 

We are seeking a registered social worker with a sound knowledge and experience of working with people with disabilities to work as part of a multi-disciplinary team.  This role will undertake assessments and care planning to support people in achieving their outcomes in the local community.   

 

Key responsibilities

  • Undertake robust assessments in conjunction with other agencies and professionals.
  • Effective commissioning of packages of support.
  • Undertake risk assessments and develop risk management plans.
  • An awareness of safeguarding and relevant legislative frameworks.  

Key Requirements

To succeed in this role you will need as a minimum:

  • Experience pre or post qualified in working with people with a disability and complex needs. 
  • Experience in risk assessment and care planning
  • Experience of effective commissioning of complex packages of support
  • Sound knowledge of legislative frameworks.
  • Ability to advocate for service user and carers and enable a social care voice. 
  •  Excellent organisational skills and structured approach.    

The post is subject to an enhanced disclosure from the DBS (previously known as CRB)

If you are newly qualified or this is your first post since qualifying then under the requirements of the Assessed and Supported Year in Employment (ASYE) you will be supported and assessed in your role as registered social worker for a period of 1 year.  Should you fail to meet the required standard and be unable to practice as a social worker the Council will have to consider the termination of your employment.
